How to Register

- Fill out the Circle K DCON Housing sheet.
- If paying by check, pay and mail it to:
Bruce Hennings at the CNH Kiwanis District Office (8360 Red Oak St. Suite 201, Rancho Cucamonga, CA 91730). - If paying by credit card you need to fill out the CC Authorization and send it in with the Housing Sheet
- Fill out and turn in at the Rancho Cucamonga Kiwanis Office the wavier and code of conduct forms.
