Club Event Report Form (CERF) - Use this form to record event hours, chairs, and other related data for your club's MRF/Hours Spreadsheet!

Club Event Report Form (CERF) 2011-2012 - This is an updated CERF, created by District Secretary Jonathan Cao-Nguyen. You do not have to use this form, you may use the old CERF if it is easier.

Kiwanis Family Report Form (KFRF) - The KFRF or the Kiwanis Family Report Form, it is a new form we are starting this year in attempts to make our Kiwanis Relations Stronger. The point of the KFRF is to inform the district of what Kiwanis family chairs all over California, Nevada, and Hawaii are doing to improve Kiwanis Family Relations. The form will be submitted to the District Kiwanis Family Chair bi-monthly. This form was made to encourage Kiwanis Family Chairs to host events such as the Shadow Program, Key to college, implement the Kiwanis Family Periods, and host any other events that will help make the Kiwanis Family closer. It is very important to implement the KFRF because it will be a contributing factor for awards at District Convention.

Medical Form - Use a variance of this form for any events you host that may require medical information from participants! Make sure to make it specific to your event!
