Monthly Report Form 2011-2012 (MRF) - The essential secretary file, fill out this form every month and send it in to the District Secretary Jonathan Cao-Nguyen in order to report your club hours and funds raised! (right click and click 'save as' to save it!)

Club Event Report Form (CERF) 2011-2012 - This is an updated CERF, created by District Secretary Jonathan Cao-Nguyen. You do not have to use this form, you may use the old CERF if it is easier.
